We also provide temporary assistance following a hospitalization to ensure that you receive your prescriptions, meal preparation, groceries while you convalesce.

TRANSITIONAL CARE



We are here to help you maintain continuity of care and ensure your needs are met. To ensure a safe and smooth transition, some of the services that our licensed nurses and certified caregivers provide include:


  • Coordination of discharge and care plans between hospitals and physicians
  • Transportation home or alternate health care setting
  • Assistance with activities of daily living (bathing, dressing, toileting, etc.)
  • Skilled nursing care
  • Medication review and reminders
  • Medication pick up at the pharmacy
  • Support to and from medical appointments
  • Assistance with household activities and responsibilities including meal preparation, housekeeping, and running errands.


RESPITE CARE


Respite Care provides short-term relief for individuals who are attending to the needs of their elderly or ill family members. With a four (4) hour minimum, services can be arranged for just an afternoon or for several days or weeks. Care can be provided at home or in a healthcare facility. At Allora Home Care, we work around your needs for self-preservation while delivering exceptional care to your loved one.
